In addition to the forms below you will also need to provide a copy of your child's certified birth certificate and a physical. All payments, forms, birth certificate and physical are required to participate as a football player or cheerleader in the NYFL.
You will need to print out all forms or request forms by contacting us. You may send your forms to: Panthers Football & Top Cat Cheer, P.O. Box 576616, Modesto CA 95357 or fax to: 551-3419, or you may bring them with you to the next sign-up date. NYFL Medical Forms must be completed by the Doctor's Office and have
